Please fill out one of the corresponding forms below (if you haven’t done so already) and send it back to us. We MUST have a form from you on file before we are able to pay you your affiliate commission. You only need to send a form in once.
Please note that you must have at least $100 in approved commissions before we are able to release your payment. If you have $100 or more in approved commissions you will be paid on the 15th of the month. If the 15th has already passed for the current month, payment will be sent the next month on the 15th.
